The SSHCO has issued a Request for Proposals (RFP) for community-based support that can help address some of the critical social determinant needs of our patient population as we work to improve health equity and well-being. Social determinants of health are conditions in which people are born, live, work and age that have an impact on good health and longevity.
Specifically, we are looking for community organization partners who focus on employment, housing, food insecurity and emergency financial support (utilities, household expenses, etc.) to apply for grants of up to $125,000 over the course of a year.
Those eligible can be any size, must be a nonprofit, need to operate in or deliver services within the service area targeted by the SSHCO, as defined in the RFP, and be able to demonstrate with metrics how they will measure performance and effectiveness.
