Community members gathered to hear about SSHCO benefits, and highlights from Phalanx – our partner providing job training and employment assistance. October 18, 2023 Town Hall

Over 200 of our community members gathered to hear directly from SSHCO patients about how being enrolled in our services has impacted them, through our healthcare services and partner resources. Over 150 of our community members gathered for a brief overview of the South Side Healthy Community Organization and progress being made toward more access to care; an update on hiring opportunities and jobs with the organization; and an opportunity to meet our new CEO, Kimberly Hobson! Community Grand Rounds

The Community Grand Rounds – a series of community events hosted by UChicago Medicine’s Center for Community Health & Vitality (CCHV), with support from the University of Chicago’s Institute for Translational Medicine and the Urban Health Initiative – welcomed SSHCO’s very own Brenda Battle, Dr. Airica Steed, Dr. Barrett Hatches and Margie Johnson to hear their thoughts on what the SSHCO means to South Side residents and answer questions from interested listeners.

Sept. 10 Listening Session Summary Report

Nearly 150 South Side healthcare providers, faith-based and community leaders, local electeds and others gathered virtually September 10, 2020 to offer their perspectives on healthcare on the South Side. Five breakout sessions produced a lot of great feedback and ideas.

Community Engagement Report

This report captures feedback from 12 virtual community listening sessions where patients, residents, hospital workers, parishioners, community organization representatives and others gathered to discuss ideas for improving health on Chicago’s South Side.

Highlights from Community-wide Survey

Hundreds of survey participants were asked, “What ideas or solutions would you like to see implemented through the South Side Health Transformation Project?” The responses to this and more were aggregated here.

Oct. 8 Summary Transformation Model

On October 8, 2020, more than 100 South Side healthcare providers, faith-based and community leaders, local elected officials and others convened to contribute their ideas in response to an early transformation model concept. The feedback from the meeting’s participants can be found in the report.

Feb. 28, 2022 Virtual Town Hall

Close to 100 South Side community members came together at a virtual update session on March 24, 2021 to hear and see an update on the South Side Health Transformation Project process and model, which has continued to evolve to reflect community input. Model Concept

Based on the feedback gathered through community listening sessions, surveys of community members, large-group virtual community meetings and more, we’re developing a model concept; a phased approach for building, transforming and scaling South Side healthcare. This model has four major components, including more access to healthcare providers in the community, targeted care programs focused on common concerns, care coordination led by community health workers who help people find the resources they need, all tied together by a technology platform meant to coordinate data and care every step of the way.
