This project is built on community input.

The South Side Healthy Community Organization has engaged more than 2,000 South Side residents, healthcare providers, faith leaders, elected leaders, patients and more in an effort to gather input and ideas on what makes a healthy community. The proposed transformation model has been informed by 12 small-group community listening sessions, two large scale community meetings, survey responses and continued community feedback.
